DOWNGRADE DISCLAIMER: Pro Fit 24, LLC requires a 15 Day notice to downgrade. If this form is filled out between the 1st-15th of the month of your request your account will reflect the change at the beginning of the next month. If your request is between the 16th-31st you will be billed one more billing cycle and then your account will reflect the change.
UPGRADE DISCLAIMER: Pro Fit 24, LLC requires a notice to switch plans. If you are upgrading your plan, you must do this in person as the pricing will have to be prorated to the new plan effective when you chose to begin the new plan. Fill this form out and submit it. We will contact you and set up a time to come into the gym to switch plans. We can also do this over the phone.
